Please print and complete the allergy evaluation forms below and bring them to your appointment at Accredited Asthma Allergy & Food Intolerance Center. These forms are required to better evaluate you and learn what has brought you to our office. Also, bring your current insurance card and driver’s license or picture identification.
Your evaluation and care are of utmost importance to us. For this reason, we see patients by appointment only and schedule only a very few (8 – 14) patients per day. As a result, we would appreciate at least a 48-hour notice of cancellation or you’ll be charged $100 for a late cancellation stating fee or no-show fee if you do not call.
We require that you pay your co-pay and fees for services not covered by your insurance at the time of your appointment. We accept cash, check, and all major credit cards. If you’re not prepared to pay, your appointment must be rescheduled.
